To consider a joint report of the Cabinet Member for Adult Care & Health, Cabinet Member for Children, Family Services & Education and the Cabinet Member for Resources, Commissioning & Governance.

 

The purpose of this paper is to provide a proposal for establishing a shadow locality board to oversee the development of the locality arrangements in Stockport that are fit for purpose to operate from 1 April 2022 within the Constitution of the Greater Manchester Integrated Care Board, and as part of the Greater Manchester Integrated Care System.

 

The Cabinet is recommended to give approval to:-

 

·         The establishment of the shadow locality system board to oversee the development of the locality arrangements in Stockport that are fit for purpose to operate from 1 April 2022 within the Constitution of the Greater Manchester Integrated Care Board, and as part of the Greater Manchester Integrated Care System. To agree the name of the shadow board as the One Health and Care Shadow Locality Board with the functions and membership as set out in section 7.1.

·         The establishment of an Executive Group to support the shadow Board as outlined in section 7.2.

·         The establishment of 5 sub-groups to drive forward the work required to build the capabilities to meet locality arrangements as set out in section 7.3.

RESOLVED – That approval be given to:-

 

·       The establishment of the shadow locality system board to oversee the development of the locality arrangements in Stockport that are fit for purpose to operate from 1 April 2022 within the Constitution of the Greater Manchester Integrated Care Board, and as part of the Greater Manchester Integrated Care System. To agree the name of the shadow board as the One Health and Care Shadow Locality Board with the functions and membership as set out in section 7.1.

A representative of the Director for Adult Social Care submitted a report (copies of which had been circulated) providing a proposal for establishing a shadow locality board to oversee the development of the locality arrangements in Stockport that would be fit for purpose to operate from 1 April 2022 within the Constitution of the Greater Manchester Integrated Care Board, and as part of the Greater Manchester Integrated Care System.

 

The Cabinet Member for Adult Care and Health (Councillor Jude Wells) attended the meeting to respond to Members’ questions.

 

The following comments were made/issues raised: -

 

·         It was noted that the report reflected streamlining services and local care and health forward locally which was a good thing.

·         Clarification was sought regarding the budget that would be available across the Greater Manchester (GM) area and whether it would be enough. In response, it was stated the budget available would be based upon the need and the money allocated to Stockport would meet the priorities identified with assurance and that the money would be spent well with the right outcomes for the people.

·         It was noted that the “Shadow Board” was intentionally kept small and focused for April 2022. There were 10 Clinical Commissioning Groups (CCGs) which would now be reduced to one main Board with everything being delegated to a local level through the Locality Board and the Sub Groups.

·         Concern was expressed regarding the positions on the Locality Board and those involved in the structure e.g. providers and involvement of the Secretary of State and GM and whether there would be a loss of control locally. In response, it was stated there would be a strong strategic voice at the GM level, together with the partnerships and representations by other bodies including the Council to support the Integrated Care System (ICS) and the model which has been voiced by the other ten GM Boards.

·         It was noted that there had been a number of challenges relating to the pandemic response, but had a very strong approach from the commissioners and built very good relationships with the care sector to respond to the challenges in the future including the funding and responding to the longer term strategy.

·         Enquiries were made relating to who the Executive Directors would be on the Board and would the positions be new or re-purposed. In response, it was stated that the Board Members would already be in position and would already exist as system leaders e.g. the Chief Executive of Stockport.

·         Clarification was sought regarding reference in the report to “optimising the workforce and sites”. In response, it was noted that the reference related to working together and not in ‘silos’ and how working together could be effective.

·         It was commented that the ambition behind the ICS was good and that it would improve the lifestyle for the staff and patients across the borough. It was stated that Stockport was not a locality, but a very large borough that was polarised and it should be reported as such.

 

The Chair commented that it was a bit disappointed that the report did not reflect the role of scrutiny in the process and would need to be included as scrutiny plays a key role through the governance process.

The following comments were made/issues raised: -

 

·         The Stockport shadow arrangements was drawn to the attention of the Board (page 46 of the report)

 

·         The Foundation Trust Board welcomed the report and has fully endorsed the recommendations and was pleased with the clarity within the report and has requested updates at future meetings of the Trust Board.

 

·         The CCG Governing Body also welcomed the report and have had a really good discussion about the opportunities to challenge and getting things right for April 2022 and the potential for new structures and doing things differently with a culture shift and collaboration and being held to account and making progress.

 

·         There was a strong view from the CCG Governing Body regarding the role of the primary care representation on the Executive Group.

 

·         Pleased to see Sector 3 representing the third sector and the importance to ensure that the voices should be heard and the worry that cultures and behaviours needed to change.

 

·         It was commented that Healthwatch was not represented on the Shadow  Board and it should be noted that it was a multi-faceted organisation and was more than just patient engagement but it was also involved in monitoring and scrutiny of the health and social care sectors.

 

·         It was suggested that ‘pooled budgets’ be looked at to benefit everyone and there should be an understanding of the implications for the parts of the system within the processes.

 

RESOLVED – That the report be noted and the recommendations be endorsed.

The Corporate Director (Corporate and Support Services) and Deputy Chief Executive submitted a report (copies of which had been circulated) setting out a proposal for establishing a shadow locality board to oversee the development of the locality arrangements in Stockport that would be fit for purpose to operate from 1 April 2022 within the Constitution of the Greater Manchester Integrated Care Board, and as part of the Greater Manchester Integrated Care System.

 

The Cabinet Member for Resources, Commissioning and Governance (Councillor Tom McGee) attended the meeting to respond to questions from the Scrutiny Committee.

 

The following comments were made/issues raised:-

 

·         A Member enquired when the five sub-groups would meet, each led by a locality Executive Director to deliver an agreed programme of work, given the relatively short period of time before NHS Stockport’s CCG functions and accountabilities would transfer to a newly Integrated Care Board in Greater Manchester from 1 April 2022.

·         Since the pooled budget started about seven or eight years ago, it had grown significantly and the aim was that Section 75 budgets would be protected.

·         The new system would have an even greater emphasis on preventative health care and a premise that hospitals were for people who were extremely ill.

·         It was clarified that there was no funding from the Government for the transition work to the new Integrated Care System.

·         With regard to the delegation from the Greater Manchester Integrated Care Board to the localities, a Member enquired what would happen if there was a disagreement. It was clarified that there was a line of accountability for NHS resources as they were transferred through national, regional and local levels.

·         It was envisaged that the appropriate Scrutiny Committee(s) of the Council would have a key role in offering checks and balances to the Locality Board post April 2022, as would be the case in the other boroughs of Greater Manchester.
