Purpose:
To approve the joint submission with Stockport Clinical Commissioning Group to the Department of Health’s ‘Better Care Fund’ (formerly known as Integration Transformation Fund).
Decision:
The Executive Meeting considered and endorsed the direction of travel detailed in the draft plan submission for the Better Care Fund (BCF) and has delegated to the Corporate Director for People, the Executive Councillor (Adult Care Services) and the Chair of the Health and Well Being Board, the authority to finalise and submit the Plan on behalf of Stockport Partners.
